This is one of a series of four Learning into Practice Project Masterclasses produced by the NSPCC and SCIE. The four masterclasses aim to introduce key ideas from the different fields and enable Serious Case Review reviewers to consider their relevance. This masterclass provides an introduction to a systems approach to reviews. Specifically, it introduces reviewers to the basic theoretical approaches to understanding the causes of error in high risk fields such as aviation, engineering and health; it highlights some key concepts from these approaches; it encourages participants to make connections between some key concepts from systems thinking and their own practice as SCR lead reviewers; and it gives participants the opportunity to consider what further support they would need in order to be able to apply systemic thinking in their reviews.

The model provides a method for getting to the bottom of professional practice and exploring why actions or decisions that later turned out to be mistaken, or to have led to an unwanted outcome, seemed to those involved, to be the sensible thing to do at the time.

The Social Care Institute for Excellence (SCIE) worked in collaboration with the London Safeguarding Children Board (LSCB) to pilot the SCIE Learning Together model for case reviews in seven London Local Safeguarding Children Boards (LSCBs). This report presents on some of the interim findings from the (LSCBs) participating in the pilots of the SCIE Learning Together model. It draws primarily on verbal feedback provided by Lead Reviewers over the course of the training, and includes direct quotations. Issues discussed include the engagement of staff and implementation issues. The report aims to share with non-participating LSCBs the experience of taking part in the pilot.
