Prevention service description

Beginning in January 2014, following a user perspective review of services, Community Team Plus is an ongoing initiative in the Stoke on Trent Area. The initiative aims to address the issues revealed by the review including a fragmentation between services, a lack of focus on what mattered to the person and unnecessary bureaucracy and expenditure.

The service, which has a strengths based approach, runs across three general practices, who have shown a commitment to integrated working. It collapses the internal assessment – provider split, so teams of community assistants, therapy assistants and social workers provide reablement as well as care management. In addition the overall purpose of the service is person focused with a strengths based approach. The overall aim of ‘Help me to help myself live well’ breaks down to include ‘Enough help to find the right sustainable solutions’ ; 'Help me build my own networks of support‘; ‘Pull expertise as needed‘; ‘Stay with me for as long as needed’ and ‘Be proportionate’.
