Explores disability and care at a national, regional and local authority level in England. The report brings together data from Census 2011, DWP and HSCIC ‘administrative data’, as well as from Wave 6 of the English Longitudinal Study of Ageing, to look at the prevalence of disability, need and care of different types, and to paint a picture of the lives of different groups. In particular, Chapter 3 provides a snapshot of disability and care in the older population in England, identifying key results. Chapter 4 looks in detail at the lives of older people with limited day-to-day activities, from their health characteristics to their living situation. Chapter 5 explores the characteristics of older people receiving unpaid and paid care including the overall adequacy of their care, as well as older people with substantial levels of disability who experience difficulty undertaking three or more ‘activities of daily living’. Chapter 6 explores the interaction of older people experiencing limited day-to-day activities with public support, i.e. disability benefits and the local authority care and support system. Chapter 7 examines the prevalence of unpaid older carers and the outcomes they experience, as well as the extent of local authority support for them. The report shows that around half of the 65+ population in England reported their day-to-day activities were limited. Of the 6.7 per cent of the older population living at home in England who reported difficulty undertaking three or more activities of daily living, around 70,000 did not receive any care, and could therefore be classed as experiencing substantial unmet need. Around 20 per cent of older carers experienced self-care (ADL) difficulties themselves.
This report evaluates the performance of government policy on care and support of older people who struggle with day-to-day activities in England during the period 2011 to 2013, using the data and insights from ‘The bigger picture: understanding disability and care in England’s older population’. Part 1 of this report examines the reach of publicly funded support; the unmet need in the older population; and variation and consistency of care and support. Part 2 considers the implications of the Care Act implementation and looks at policy development beyond 2016, focusing on eligible needs after the Act, financial eligibility and the means test after 2016 and mapping, identifying and engaging older population groups. The report concludes that given the feasibility and budget challenges implied by the sheer numbers of older people experiencing difficulties with activities of daily living, a rethink and revolution is required among national and local policymakers around how individuals and families are engaged and supported. This will mean revisiting the balance between consistency and variation in services organised by local authorities, as well as fully integrating and exploiting the different ‘touch points’ and ‘gateways’ available for engaging the older population. It will also mean evaluating which aspects of the vision of the Care Act need to be fulfilled by local authorities directly, or can be devolved to empowered, third-party charities and organisations at a local level.

This independent economic evaluation of the British Red Cross Support at Home service focuses on four services which were found to improve outcomes in an earlier British Red Cross evaluation. The services all aim to help people to build their confidence and regain their independence during times of particular difficulty.Those evaluated were 'Next Steps', where volunteers provide home visits and monitor how people are coping following hospital discharge; 'Care in the Home' services delivered by staff and volunteers providing social visits, support and help with household tasks; and a Neighbourhood/Community service in Scotland which focused on linking people to existing services and volunteer-led services such as befriending. The final sample for this analysis consisted of a total of 52 people, the majority of who were over 65. Two outcomes were used in the economic analysis: an increased ability to manage daily activities and improved wellbeing. The evaluation identified cost savings that were related to a reduced need for formal/ informal care and general help around the home; a reduced risk of falls and malnutrition, particularly amongst those with unmet care needs; and, to a lesser degree, a reduced need for treatment of depressive symptoms. The total savings identified amounted to more than five times the cost of the service. The average cost of the intervention was £169 per person (based on the services and sample data in the Red Cross evaluation) and the identified savings came to £880 per person.
This paper explores responses to changes arising from bodily frailty observed among older people participating in the AKTIVE study and discussed with them during research visits. Focusing on older people living at home with different types of frailty, the AKTIVE project aimed both to enhance understanding of how they (and those supporting them) accessed, engaged with and used the telecare equipment supplied to them, and to explore the consequences for them of doing so. This paper identifies which daily activities were affected in older age and the strategies older people drew upon to cope. The paper also explores how telecare was combined with other support mechanisms to help older people maintain both practical and recreational daily activities. Throughout, there is discussion about limitations in how care support was sometimes provided, including how telecare was acquired and used by older people and/or those caring for or supporting them, and how far these problems might be overcome by more proactive implementation.

An evaluation of the British Red Cross Support at Home services, which provide time-limited care and support to people at a time of crisis who are finding it difficult to cope at home. Overall the research highlighted that the common area of major impact of Support at Home is the enhancement of service users’ quality of life. The support provided is characterised by a strong sense of trust by service users in the Red Cross brand alongside a compassionate, caring, non-judgemental, time-flexible and person-enabling approach. In particular, the findings show that four service user outcomes were significantly improved or increased following receipt of support. These include: improved wellbeing; increased ability to manage daily activities; increase in leisure activities; and improved coping skills. Other positive changes were also reported related to the wider benefits of the service beyond the service user outcomes alone, including enabling safe discharge, supporting carers and enabling patient advocacy. The report identifies a series of action points to help further develop the services: champion Red Cross strengths, respond to the changing profile of service users, develop active partnerships to extend reach and maximise impact, clarify the Red Cross’ position for people in need who fall outside of commissioned contracts, collect consistent and routine local and national data to inform service learning and development, develop signposting to ensure long-term impact and grow skills in order to advocate on behalf of service users.
