A guide for primary care commissioners in NHS England and Clinical Commissioning Groups (CCGs) on how to involve patients, service users, carers and the public in the commissioning of primary care services. This includes involving throughout the commissioning process in the planning, policy making, buying and monitoring primary care services such as general practice, community pharmacy, dental, and eye health services. The Framework looks at how NHS England involves people in commissioning at national and local levels; co-commissoning with CCGs; and individual responsibilities as a policy or commissioning manager in primary care. It also includes short case study examples, provides details of additional sources of information, and key partner organisations and networks with an interest in public participation. It has been co-designed with members of the Working Group for Patient and Public Participation in Primary Care Commissioning and also incorporates feedback received from key stakeholders. The document will also be of interest to patients and the public, the voluntary sector, and providers of health and social care services. It is designed to be read with the NHS England Patient and Public Participation Policy and the Statement of Arrangements and Guidance on Patient and Public Participation in Commissioning.
This paper sets out research to understand and work within the emerging landscape in which organisations find that the people they serve acquire greater prominence among their multiple stakeholders and power gradients have to adjust accordingly. Respect for the dignity of people and how organisations respond to their needs, wishes, gifts and aspirations are becoming pivotal. Drawing on a survey and participation of 20 sector leaders at a roundtable hosted by the University of Birmingham and funded by the Barrow Cadbury Trust, the research proposes a framework for organisations to review and, if necessary, restate the priority given to people (commonly called users), develop practice that encourages reciprocity in the design and delivery of products and services, and establish processes that are pervious and accountable to people and their networks. At the core of the framework is the concept of contingent capacity. Contingent capacity is purposeful, distributive and empowers workers (staff and volunteers), to listen to and respect people and, inspire their participation. The approach comprises three stages, which include: Purpose and Power – against a backdrop of more assertive citizens and a changing socio-economic environment, reviewing how the organisation continues to respect the dignity of the people it serves and ensures they are able to contribute to decisions that affect them; Reciprocal Engagement – recalibrating practice and culture to give greater priority to people and encourage deep and iterative engagement; and Outcome Plus – ensuring processes optimise value, not just to the organisation but, also, to the people and the wider communities around them.
This guide evaluates the experience of involving older people in a research study that explored the age-friendliness of three areas of Manchester. It offers practical tips and critical reflections to help rethink how older people can be involved in research and social action to improve the physical and social environment of their neighbourhood. For the project a group 18 older residents were recruited and trained in designing interview questions, interviewing, data collection, and sharing the findings. The guide outlines the aims of the study, the methodology of the research and a summary of research activities undertaken. It then covers: what 'age-friendly means'; the co-researchers' motivations to participate in the study; the advantages and challenges of involving older residents; skills and knowledge acquired through the project; key findings; and suggested improvements to the age-friendliness of neighbourhoods. The guide includes contributions from older co-interviewers and representatives of community organisations who were involved in the project. The guide concludes by suggesting three principles for developing age-friendly neighbourhoods: that they should empower older people and enable social participation; they are a reminder about the rights of all citizens to full use of resources in their neighbourhood; and the importance of recognising both the social and physical dimensions which make up age-friendly communities.
HOUSING LEARNING AND IMPROVEMENT NETWORK
This practice briefing outlines the work being undertaken to capture examples of practice of resident-led housing for older people, building on the 2013 report ‘Growing older together: the case for housing that is shaped and controlled by older people’. This key area of work recognises that there is a need to increase the supply of housing suitable for older people and ensure that the offer of new housing options and choices prove attractive to a new generation of older people with changing priorities and higher expectations. The work programme will comprise several elements, including: an overall report, covering a broad range of examples; liaison with community-led housing organisations, older people’s representative forums/networks and other interested parties; and a spring conference for 2016.
An evaluation of a Skills for Care funded a programme of 12 pilot projects, across 11 organisations, for 12 months in 2013/14 designed to support the development of dementia friendly communities (DFCs), by improving community understanding and awareness of dementia and supporting people living with dementia and their carers to participate in their communities. Section 1 of the report provides an introduction both to the underlying objectives of the programme, and to the participating pilot sites. Section 2 presents an overview of the cross-cutting themes and issues identified across the sites, including motivation and engagement, working with the wider community, intergenerational aspects, engaging with GPs and the NHS, and impact and outcomes. The methodology for the evaluation included analysis of written reports; and one to one semi-structured interviews with project leads. The report highlights the importance of motivation and personal engagement as driving forces while suggesting that most projects encountered difficulties – to a greater or lesser extent – in trying to work with the wider community in developing awareness and understanding of dementia. A few of the projects were addressing intergenerational dimensions of dementia awareness and were working with schools, or were planning to develop such work. In working with a range of local partners many projects were deliberately engaging with the NHS in general and with GPs in particular to increase diagnosis rates. The report concludes that equipping people with the skills and understanding to respond to the needs of people with dementia has great potential to bring about transformational change and to enable genuine social inclusion.

This report presents the main findings from a collaborative evaluation over a year with four small housing associations who decided in 2013 to work towards becoming dementia friendly organisations. The report illustrates the changes the associations made and gives their reasons for making those changes. It describes detailed experiences of the small housing associations, reflecting on their starting points and examining the methods they used to establish dementia friendly housing organisations. The report looks at the reasons why leaders and key staff decided to invest time and energy into becoming dementia friendly housing organisations. It also looks at the reasons why training was so important and at how the organisations started to change their practices, processes and policies. Drawing on a variety of measures to compare and contrast the work of the associations against, the report concludes with an overall evaluation, showing that through a mix of pragmatism and planning the housing associations were able to make deep changes.

It also draws out seven major themes from the research covering service design, the role of carers, the need for regular assessment, and the importance of social interaction. The Active at 60 Community Agents programme was a Department for Work and Pensions fund to encourage community groups and their volunteers to help people approaching and post retirement (particularly those at risk of social isolation and loneliness in later life) to stay or become active and positively engaged with society. It was launched in March 2011 and ran until December 2011. This evaluation of the programme included surveys and interviews with local funders, group leaders, community agents (volunteers whose role aimed to empower and support older people to become and/or stay active) and older people. The report describes the background and methodology of the study and presents the findings, covering the role of Community Agents, reaching and engaging older people, what groups did with the funding, what difference the programme made to older people who took part and wider benefits, the legacy of the programme, and the role of local funders and programme management. It also discusses how far the programme achieved its aims and sets out key lessons learned.
