Stoke on Trent City Council
Community Team Plus involves multidisciplinary health and care teams supporting people across six Stoke on Trent localities to 'help me to help myself to live well'. They are tasked with being accessible, creative, resourceful and helpful.
"Before I joined Mates and Dates I didn’t go out in the evening unless it was a family occasion. Now I can look forward to going out once a month and to dates with my girlfriend in-between. It gives me something to look forward to.”

Wigan Borough Clinical Commissioning Group and Wigan Council
The Wigan Community Link Worker service provides person centred support that enables individuals to access community activities keep them independent, whilst taking greater control of their health and wellbeing, and connecting them to their communities. The service was jointly commissioned by Wigan Borough Clinical Commissioning Group and Wigan Council with the aim of improving the health and wellbeing of local people through better connections to appropriate sources of support in the community. Initially piloted in 2015, run by City Health Care Partnership (CHCP), with 11 practices the service has grown and now covers the whole Borough (63 practices). In March 2016, funding for the service was extended for a year.
South West London & St George’s Mental Health NHS Trust
Led by South West London and St George’s Mental Health NHS Trust. This project implemented GLOW (Graduated Living Skills outside the Ward) within adult community mental health services in the London boroughs of Kingston and Richmond. This intervention aimed to improve outcomes, social functioning and quality of life of people with affective and personality disorders though supporting individuals to regain their independence by both showing self-care and participating in their wider community

‘16’ is service that blends tried and tested supported employment approaches with person centred approaches in order to achieve employment focused outcomes for a range of groups.
Jobs Friends Houses
Founded by Sgt Steve Hodgkins, Jobs, Friends and Houses offers volunteering, training and employment opportunities to marginalised adults, mainly ex offenders and/or those with mental health issues or in abstinence. The project is built on the belief that people will make make stable and sustainable changes when they have:
"• a safe place to live that is free from threat
• a social network that is supportive of their attempts at recovery
• meaningful activities, particularly those that confer self-worth and that offer a positive future"
Call and Check is a Jersey based initiative which uses the delivery platform of the postal service to support people to live independently in their own homes. Postal workers are trained to make short visits to residents which involve conversations covering health and social wellbeing topics. The service links residents with health social care and voluntary sector providers whilst also preventing social isolation.
Age UK Sutton
One Voice for Age was founded, in 2002, to ensure that the views of older people in Sutton, were represented and heard, and, in turn, that these voices aid the review of existing services and influence the development of services for older people
The initiative relies on a strong base of 40 – 50 volunteers (or members as the project calls them) who are trained to interview other older people. Some of these older people may be isolated due to ill health or disability and, without this active involvement and outreach, would not usually have their voice heard. These interactions are then applied in order to campaign on behalf of older people and represent their views to local policy and decision makers.
Originally run as a pilot, Community Circles began in 2012 and are now located throughout the UK providing support in the community, by the community, helping people to achieve their goals in a way and place that suits them – from living rooms to care homes. Everyone in a circle gains by being part of something shared, focused and often life changing.
