Specialist care - End of life care: Ideas you could use

Developing the role of Dementia Voice Nurse

Housing 21 runs a dementia specialist unit in Westminster —Tresham Resource Centre

This service identified a need for specialist care for people with dementia who are dying and want to remain in their own homes. Working with key professionals and additional funding they developed the role of Dementia Voice Nurse – a qualified nurse with expertise in dementia, end of life care, symptom control and effective communication to primarily act as an expert practitioner, advocate and facilitator. The Dementia Voice Nurse will liaise with external agencies and provide a consistent link with GPs and other primary healthcare practitioners. (see Dutton)
