  This suite of audit tools is designed to evaluate the implementation of MCA in the hospital discharge planning process. It includes tools to audit policies and procedures and clinical records, and questionnaires to elicit feedback from staff, patients, patients’ relatives, carers and advocates, and recipient organisations such as care homes.
