Fluctuating needs: the Care Act 2014

Under the Care Act 2014, assessments should reflect more accurately a comprehensive picture of people's needs - including how they change over time. In this film two people, one with mental health needs, the other with a physical disability, talk about their conditions, assessment, how their needs can fluctuate and the impact this has on the level of care and support they need. The film illustrates how the new requirement aims to recognise people as individuals by endorsing a much-needed degree of flexibility and responsive care, as well as offering valuable support for people with mental health and physical health conditions which may vary over time.

Better life for older people with high support needs: the role of social care

This briefing summarises the seven challenges set out by the Joseph Rowntree Foundation’s (JRF) A Better Life programme which will help older people with high support needs achieve a better quality of life. It identifies key SCIE resources that will support people working in social care address each of the challenges in their practice and relationships with older people with high support needs and achieve the overall aim of supporting choice, control and quality in their lives. The challenges cover: encouraging positive images of ageing; taking a person centred approach to care; encouraging importance of meaningful relationships; treating older people as equal stakeholders in their care and support services; allowing older people with high support needs to make decisions; and being open to innovative and new approaches of providing care and support.

End of life care: why talking about death and dying matters

A personalised approach gives people moving towards the end of life a voice and influence over their care. This video for health and care staff looks at how personalised care can be achieved through the Living Well tool, which involves creating a one page profile to record a person’s needs and wishes.
