This is one of a series of four Learning into Practice Project Masterclasses produced by the NSPCC and SCIE. The four masterclasses aim to introduce key ideas from the different fields and enable Serious Case Review reviewers to consider their relevance. This masterclass provides an introduction to a systems approach to reviews. Specifically, it introduces reviewers to the basic theoretical approaches to understanding the causes of error in high risk fields such as aviation, engineering and health; it highlights some key concepts from these approaches; it encourages participants to make connections between some key concepts from systems thinking and their own practice as SCR lead reviewers; and it gives participants the opportunity to consider what further support they would need in order to be able to apply systemic thinking in their reviews.
