This report summarises discussions from a roundtable event attended by older people and carer representatives, practitioners, providers and commissioners to identify how the NICE guideline on supporting older people with multiple long-term conditions and their carers could best be used and implemented. It also sets out practical examples, actions and ideas to help improve local practice. Small groups discussed how the guideline can help achieve three priorities that the Guideline Committee identified as most important for potential impact and the likely significant challenges. These were: empowering older people and carers; empowering health and social care practitioners; and integration of different care and support options to enable person-centred care. Suggested actions and practice examples in each of the three priority areas.

This report argues that giving citizens greater control over their health and care can both promote the redesign of services, so that they are developed around citizens needs and aspirations, and also save money by supporting people to manage their conditions themselves. The report begins by looking at what empowerment in health and care means and the benefits it can bring in terms of autonomy, better health outcomes, patient satisfaction, and reductions in cost. It then describes previous programmes and initiatives which aimed to give citizens and communities greater power and why these approaches have not been entirely successful. It then describes five models of care which actively empower citizens and communities and address the deficiencies of previous initiatives. The models described are: social prescribing; brokerage and integration; peer support; asset-based community development; and technology-enabled care plans, which provide people with the tools to better manage their condition themselves. The final chapter identifies five enablers of systems change to help encourage the development and wider adoption of these new models of care: finance, devolving power and integration, recruitment and training workforce, the adoption new technology, empowering citizens to have greater control of their health and care.

The Active at 60 Community Agents programme was a Department for Work and Pensions fund to encourage community groups and their volunteers to help people approaching and post retirement (particularly those at risk of social isolation and loneliness in later life) to stay or become active and positively engaged with society. It was launched in March 2011 and ran until December 2011. This evaluation of the programme included surveys and interviews with local funders, group leaders, community agents (volunteers whose role aimed to empower and support older people to become and/or stay active) and older people. The report describes the background and methodology of the study and presents the findings, covering the role of Community Agents, reaching and engaging older people, what groups did with the funding, what difference the programme made to older people who took part and wider benefits, the legacy of the programme, and the role of local funders and programme management. It also discusses how far the programme achieved its aims and sets out key lessons learned.
