Introducing ConnectWELL - a social prescribing service – initially funded and piloted in 2014 by NHS Rugby CCG, which aims to improve health and wellbeing for patients and clients. ConnectWELL provides Health Professionals with just one, straightforward referral route to the many Voluntary and Community Sector organisations, groups and activities that can address underlying societal causes, manage or prevent compounding factors of ill-health. ConnectWELL has over 900 organisations and activities, ranging from Carers’ support, community groups, disability services, Faith / Religious / Cultural Activities, Housing / Homelessness Support, Mentoring, Music Groups, and volunteering opportunities.

The Kinect Project: group motion-based gaming for people living with dementia

Engaging in enjoyable activities is an essential part of well-being, but people with dementia can find participation increasingly difficult. Motion-based technologies can provide meaningful engagement in a wide range of activities, but for people with dementia to take advantage of these devices requires a good understanding of how best to select and present these activities to this population. The objective of this study was to explore the use of motion-based technology (Xbox Kinect) as a group activity for people with dementia who attend adult day programmes. This qualitative study took place in an adult day programme for older adults with age-related challenges. Participants (n = 23) were observed while playing a digital bowling game presented on Xbox Kinect one hour per week for a period of 20 weeks, to capture naturalistic data. Field notes generated through observations were transcribed and analysed to identify emerging themes. The findings revealed three predominant themes which illustrate the potential of motion-based technology as a group activity for people with dementia who attend adult day programmes: (a) the importance of having a trained trainer, (b) learning versus mastery and (c) playing ‘independently together’. People with dementia can learn to play games presented on motion-based technology and enjoy doing so. Furthermore, using the technology in a group setting fostered an encouraging and supportive environment which further contributed to the leisure experience. However, to be used most effectively, staff must be trained to set-up and interact with the technology, as well as introduce, teach and support people with dementia to use it.

An evidence summary of health inequalities in older populations in coastal and rural areas: full report

A rapid evidence review on the health inequalities experienced by older people in coastal and rural areas, supplemented with case studies. The review aimed to identify key determinants of health inequalities experienced by older populations in coastal and rural areas; looks at the strengths and assets of ageing populations in these areas; reviews the effectiveness health and social care system interventions and whole system approaches; and assess the opportunities for using digital technology. In addition to a rapid review of the national literature (covering the UK and Ireland), a search of international literature focusing on interventions to reduce health inequalities was also carried out. It is intended for local authorities, clinical commissioning groups and other health and care organisations to inform strategic planning, service design and commissioning, as well as the development of local community infrastructure. It also provides summary of key considerations in taking an asset-based approach in reducing inequalities and promoting productive healthy ageing in these areas. Two accompanying reports have also been published, an executive summary with main messages and an annex of included studies.

Dance to Health 'Phase 1 roll-out [test and learn]' evaluation: first report

An evaluation of the Dance to Health falls prevention dance programme. The evaluation investigates whether Dance to Health is an effective and cost-effective way to address older people’s falls and whether the programme helps older people in danger of falling overcome lost confidence, reduced independence and increased isolation. The evaluation used a mixed methods approach that included quantitative, qualitative and econometric research. Primary outcome measures included the impact on falls, positive side-effects (mentally and physically), patient pull, attendance and adherence, fidelity to existing falls prevention programmes and cost effectiveness. The findings show that Dance to Health is helping older people in danger of falling overcome lost confidence, reduced independence and increased isolation. There was a 44 percent reduction in the number of falls, positive improvements in participants' physical and mental wellbeing, including improved Timed Up and Go (TUG) times and reduced fear of falling. Additionally, based on the analysis conducted, there is a potential cost saving of over £149m over a 2-year period, of which £120m is a potential cost saving for the NHS. Dance to Health's fidelity to existing physiotherapy programmes was also confirmed. The evidence suggests that Dance to Health offers the health system a more effective and cost-effective means to address the issue of older people's falls.

User requirements for technology to assist aging in place: qualitative study of older people and their informal support networks

BACKGROUND:Informal support is essential for enabling many older people to age in place. However, there is limited research examining the information needs of older adults' informal support networks and how these could be met through home monitoring and information and communication technologies. OBJECTIVE: The purpose of this study was to investigate how technologies that connect older adults to their informal and formal support networks could assist ageing in place and enhance older adults' health and well-being. METHODS: Semistructured interviews were conducted with 10 older adults and a total of 31 members of their self-identified informal support networks. They were asked questions about their information needs and how technology could support the older adults to age in place. The interviews were transcribed and thematically analysed. RESULTS: The analysis identified three overarching themes: (1) the social enablers theme, which outlined how timing, informal support networks, and safety concerns assist the older adults' uptake of technology, (2) the technology concerns theme, which outlined concerns about cost, usability, information security and privacy, and technology superseding face-to-face contact, and (3) the information desired theme, which outlined what information should be collected and transferred and who should make decisions about this. CONCLUSIONS: Older adults and their informal support networks may be receptive to technology that monitors older adults within the home if it enables ageing in place for longer. However, cost, privacy, security, and usability barriers would need to be considered and the system should be individualizable to older adults' changing needs. The user requirements identified from this study and described in this paper have informed the development of a technology that is currently being prototyped.

Acceptance and use of innovative assistive technologies among people with cognitive impairment and their caregivers: a systematic review

Cognitive impairments (CI), associated with the consequences of Alzheimer’s disease and other dementias, are increasingly prevalent among older adults, leading to deterioration in self-care, mobility, and interpersonal relationships among them. Innovative Assistive Technologies (IAT) such as electronic reminders and surveillance systems are considered as increasingly important tools to facilitate independence among this population and their caregivers. The aim of this study is to synthesise knowledge on facilitators and barriers related to acceptance of and use of IAT among people with CI and their caregivers. This systematic review includes original papers with quantitative, qualitative, or mixed methods design. Relevant peer-reviewed articles published in English between 2007 and 2017 were retrieved in the following databases: CINAHL; PubMed; Inspec; and PsycINFO. The Mixed Method Appraisal Tool (MMAT) was used for quality assessment. We retrieved thirty studies, including in total 1655 participants from Europe, USA/Canada, Australia, and Asia, enrolled in their homes, care-residences, day-care centres, or Living Labs. Two-thirds of the studies tested technologies integrating home sensors and wearable devices for care and monitoring CI symptoms. Main facilitators for acceptance and adherence to IAT were familiarity with and motivation to use technologies, immediate perception of effectiveness (e.g., increase in safety perceptions), and low technical demands. Barriers identified included older age, low maturity of the IAT, little experience with technologies in general, lack of personalization, and support. More than 2/3 of the studies met 80% of the quality criteria of the MMAT. Low acceptance and use of IAT both independently and with caregivers remains a significant concern. More knowledge on facilitators and barriers to use of IAT among clients of health care and social services is crucial for the successful implementation of innovative programmes aiming to leverage innovative technologies for the independence of older people with CI.

Physical activity programs for older people in the community receiving home care services: systematic review and meta-analysis

The proportion of older adults is increasing around the world and most wish to live in their home until they die. To achieve this, many will require services in the home to remain living independently. To maintain function (ie, strength, balance, and endurance), physical activity needs to be undertaken on a regular basis, and is essential as a person ages. Unfortunately, as people age there is a tendency to reduce activity levels, which often leads to loss of function and frailty, and the need for home care services. This updated systematic review includes a mix of study methodologies and meta-analysis, and investigated the effectiveness of physical activity/exercise interventions for older adults receiving home care services. Eighteen studies including ten randomized controlled trials meeting the selection criteria were identified. Many of the studies were multi-factorial interventions with the majority reporting aims beyond solely trying to improve the physical function of home care clients. The meta-analysis showed limited evidence for effectiveness of physical activity for older adults receiving home care services. Future exercise/physical activity studies working with home care populations should consider focusing solely on physical improvements, and need to include a process evaluation of the intervention to gain a better understanding of the association between adherence to the exercise program and other factors influencing effectiveness.

An evaluation of Rainbow Services community builder project: the Senior Safe and Social Programme

An evaluation of the Senior Safe and Social Project in Harlow, a project to reduce loneliness and social isolation in older people. The evaluation aims to provide an insight into why the model is working well, whether it could be applied to other demographics, how the approach compares with similar models, and the sustainability of the project. The project, based on an Asset Based Community Development (ABCD) model, provides weekly clubs and events in local communities, which are directed by older people themselves. The evaluation identified key factors identified that helped the model work. These included that social groups are design designed around the participants and give them control over what they do. Volunteers’ involvement in the project represents a reciprocal exchange, giving the volunteer the opportunity to socialise and sense of usefulness and connection in the local community. The evaluation suggests that the model could be applied to other locations and other age groups, such as young mothers and ‘empty nesters’. The report includes suggestions for the future development of the project.

Electronic assistive technology for community-dwelling solo-living older adults: a systematic review

The proportion of older adults who live alone in single households is growing continuously. In the care of these solo-living older adults, electronic assistive technology (EAT) can play an important role. The objective of this review is to investigate the effects of EAT on the wellbeing of community-dwelling older adults living alone in single households. A systematic review of English articles was conducted based on PMC, Scopus, Web of Science and the Cochrane database. Additional studies were identified from the references. In total, 16 studies were identified, six of them with follow-up. There is evidence that EAT can improve the physical and mental wellbeing of older adults. There was little evidence that EAT can improve social wellbeing. We conclude that more personalized designs and interventions, and more user engagement could be embedded in the design of EAT for solo-living community-dwelling older adults and that more evidence is needed regarding the effects of those interventions.

How do “robopets” impact the health and well‐being of residents in care homes? A systematic review of qualitative and quantitative evidence

BACKGROUND: Robopets are small animal-like robots which have the appearance and behavioural characteristics of pets. OBJECTIVE(S): To bring together the evidence of the experiences of staff, residents and family members of interacting with robopets and the effects of robopets on the health and well-being of older people living in care homes. DESIGN: Systematic review of qualitative and quantitative research. DATA SOURCES: This study searched 13 electronic databases from inception to July 2018 and undertook forward and backward citation chasing. METHOD(S): Eligible studies reported the views and experiences of robopets from residents, family members and staff (qualitative studies using recognised methods of qualitative data collection and analysis) and the effects of robopets on the health and well-being of care home residents (randomised controlled trials, randomised crossover trials and cluster randomised trials). Study selection was undertaken independently by two reviewers. This study used the Wallace criteria and the Cochrane Risk of Bias tool to assess the quality of the evidence. This study developed a logic model with stakeholders and used this as a framework to guide data extraction and synthesis. Where appropriate, meta-analysis were used to combine effect estimates from quantitative studies. RESULT(S): Nineteen studies (10 qualitative, 2 mixed methods and 7 randomised trials) met the inclusion criteria. Interactions with robopets were described as having a positive impact on aspects of well-being including loneliness, depression and quality of life by residents and staff, although there was no corresponding statistically significant evidence from meta-analysis for these outcomes. Meta-analysis showed evidence of a reduction in agitation with the robopet "Paro" compared to control (-0.32 [95% CI -0.61 to -0.04, p = 0.03]). Not everyone had a positive experience of robopets. CONCLUSION(S): Engagement with robopets appears to have beneficial effects on the health and well-being of older adults living in care homes, but not all chose to engage. Whether the benefits can be sustained are yet to be investigated. IMPLICATIONS FOR PRACTICE: Robopets have the potential to benefit people living in care homes, through increasing engagement and interaction. With the robopet acting as a catalyst, this engagement and interaction may afford comfort and help reduce agitation and loneliness.
