Community services: what do we know about quality? Briefing

This briefing looks at trends in national measures in English community trusts to try to gain a view of quality in community services more generally. The analysis examines trends in routinely collected national quality measures in 18 community trusts in England, which account for a quarter of all community health services delivered in the NHS. Some of the key findings are: care in community trusts was predominantly delivered by professionally qualified clinical staff such as community health nurses, allied health professionals and community health visitors – staff numbers in the 18 trusts stayed roughly stable between late 2013 and 2016, although demand has almost certainly increased; these staff were roughly as satisfied with their jobs as staff in all NHS trusts, although they were less likely to recommend their trust as a place to work; the median waiting time for an outpatient appointment was three days longer in the community than across all trusts in England; patients using services offered by community trusts would generally recommend them to a friend and were less likely to experience harm compared to those using services provided by non-community trusts. The briefing concludes that the difficulties experienced in gathering useful information on community services indicate that the national lack of community data needs to be resolved before questions of quality can be meaningfully answered.

Creating a better care system: setting out key considerations for a reformed, sustainable health, wellbeing and care system of the future

In this report, commissioned by the Local Government Association, a journey towards better health and care for individuals is set out; driven by local system leaders and supported by a more empowering and enabling system. The report has been developed through: a review of existing literature published by partners, charities and research organisations; four workshops with the LGA and partners to define the vision, understand the system barriers from a range of perspectives and describe the required changes; and further discussion with regional contacts and the Health Transformation Task Group to sense check that barriers and key considerations are locally relevant and reflect the experience in local areas. Section 1 sets out a vision for better care and support, arguing that a reformed system needs to deliver: better health and wellbeing more equally enjoyed; better choice and control for all; better quality care, tailored for each person; and better outcomes for each pound spent. Section 2 focuses on key barriers preventing the achievement of a reformed system. These include: creating dependency through the way treatment is provided; chronic underfunding of the system and a lack of capacity to transform; fragmented commissioning incentivising treatment over demand management; and national regulations that disempower local areas. Section 3 sets out four steps to better care, which are: put people in control; fund services adequately and in an aligned way; devolve power to join up care, support and wellbeing; and free the system from national constraints. The report concludes that collectively these steps will enable localities to address challenges, deliver a better system and ultimately drive better outcomes and greater sustainability for all.

Commissioning for better outcomes: a route map

Sets out standards for high quality commissioning to support a dynamic process of continuous improvement and, through self-assessment and peer review, to challenge commissioners and their partners, to strengthen and innovate to achieve improved outcomes for adults using social care, their carers, families and communities. There are 12 standards grouped into four domains, including person-centred and outcome-focused commissioning, inclusiveness, effective leadership and promotion of sustainable and diverse market place. They have been developed from a review of the available literature, the engagement of a wide range of stakeholders, the input from a project steering group coordinated by Think Local Act Personal, and an expert review of a final draft of the standards by local authorities and other key organisations. The prototype document will be piloted by a small number of local authorities and will shape and inform a new offer within the Local Government Association peer challenge programme which will become available in April 2015.
