The Community Navigator Study: results from a feasibility randomised controlled trial of a programme to reduce loneliness for people with complex anxiety or depression

Background: Loneliness is common among people with mental health problems and predicts poorer recovery from depression and anxiety. Needs for support with loneliness and social relationships are often under-addressed in mental health services. The Community Navigator programme was designed to reduce loneliness for adults (aged 18 and above) with complex depression or anxiety who were using secondary mental health services. Acceptability and feasibility of the programme and a trial evaluation were tested in a feasibility randomised controlled trial with qualitative evaluation. Methods: Forty participants with depression or anxiety using secondary mental health services were recruited from mental health services in two London sites and randomised to receive: the Community Navigator programme over six months in addition to routine care (n = 30); or routine care (n = 10). Measures of loneliness, depression, other clinical and social outcomes and service use were collected at baseline and six-months follow-up. Levels of engagement in the programme and rates of trial recruitment and retention were assessed. Programme delivery was assessed through session logs completed by Community Navigators. The acceptability of the programme was explored through qualitative interviews (n = 32) with intervention group participants, their family and friends, programme providers and other involved staff. Results: Forty participants were recruited in four months from 65 eligible potential participants asked. No one withdrew from the trial. Follow-up interviews were completed with 35 participants (88%). Process records indicated the programme was delivered as intended: there was a median of seven meetings with their Community Navigator (of a maximum ten) per treatment group participant. Qualitative interviews indicated good acceptability of the programme to stakeholders, and potential utility in reducing loneliness and depression and anxiety. Conclusions: A definitive, multi-site randomised controlled trial is recommended to evaluate the effectiveness and cost-effectiveness of the Community Navigator programme for people with complex anxiety and depression in secondary mental health services.

Social prescribing evidence map: summary report

Summary findings from an evidence mapping to share evidence on the effectiveness and practice of social prescribing to improve the health and well-being of individuals with social, emotional or practical needs. Sixty-two sources met the inclusion criteria for the evidence map. Two main types of social prescribing were identified. Those targeting psychosocial needs and those intended for people who are sedentary and/or overweight or obese. In total six types of social prescribing initiative are described: link worker programmes (schemes linking people to a facilitator who assessed them and referred them on to community support); community arts programmes; a horticultural programme; exercise referral schemes; commercial weight loss programmes and referral to welfare rights advice. What approaches to social prescribing work, for whom, and in what circumstances? A realist review

The use of non‐medical referral, community referral or social prescribing interventions has been proposed as a cost‐effective alternative to help those with long‐term conditions manage their illness and improve health and well‐being. However, the evidence base for social prescribing currently lags considerably behind practice. In this paper, we explore what is known about whether different methods of social prescribing referral and supported uptake do (or do not) work. Supported by an Expert Advisory Group, we conducted a realist review in two phases. The first identified evidence specifically relating to social prescribing in order to develop programme theories in the form of ‘if‐then’ statements, articulating how social prescribing models are expected to work. In the second phase, we aimed to clarify these processes and include broader evidence to better explain the proposed mechanisms. The first phase resulted in 109 studies contributing to the synthesis, and the second phase 34. We generated 40 statements relating to organising principles of how the referral takes place (Enrolment), is accepted (Engagement), and completing an activity (Adherence). Six of these statements were prioritised using web‐based nominal group technique by our Expert Group. Studies indicate that patients are more likely to enrol if they believe the social prescription will be of benefit, the referral is presented in an acceptable way that matches their needs and expectations, and concerns elicited and addressed appropriately by the referrer. Patients are more likely to engage if the activity is both accessible and transit to the first session supported. Adherence to activity programmes can be impacted through having an activity leader who is skilled and knowledgeable or through changes in the patient's conditions or symptoms. However, the evidence base is not sufficiently developed methodologically for us to make any general inferences about effectiveness of particular models or approaches.

Local Area Coordination: summative evaluation

The results from the third phase of an evaluation of a Local Area Coordination approach developed in York, which involved the introduction of three Local Area Coordinators. The evaluation aimed to identify early outcomes at the level of individuals, families, community and system (including project objectives and cost effectiveness). It also aimed to identify emerging insights and potential future opportunities for data collection, and longitudinal approaches to Local Area Coordination outcomes over a longer timeframe. Methods used included analysis of performance data, review of documentation and interviews with Local Area Coordinators, Programme Managers and community organisations. The findings show that Local Area Coordination is operating as intended, and is providing support to people not previously known to services. People also welcome long-term focus of Local Area Coordinator work. It also identified examples of real changes as a consequence of Local Area Support, including preventative interventions and helping families navigate complex and challenging circumstances.

Service-users’ perspectives of link worker social prescribing: a qualitative follow-up study

Background: Social prescribing enables health-care professionals to address non-medical causes of ill-health by harnessing the resources of the voluntary and community sectors in patient care. Although increasingly popular in the UK, evidence for the effectiveness of social prescribing is inconclusive and longer-term studies are needed. This study aimed to explore experiences of social prescribing among people with long-term conditions one to two years after their initial engagement with a social prescribing service. Methods: Qualitative methods comprising semi-structured follow-up interviews were conducted with 24 users of a link worker social prescribing service who had participated in an earlier study. Participants were aged between 40 and 74 years and were living in a socioeconomically-deprived area of North East England. Results: Participants reported reduced social isolation and improvements in their condition management and health-related behaviours. However, many participants had experienced setbacks, requiring continued support to overcome problems due to multi-morbidity, family circumstances and social, economic or cultural factors. Findings indicated that, in this sample of people facing complex health and socioeconomic issues, longer-term intervention and support was required. Features of the link worker social prescribing intervention that were positively appraised by participants, included a highly personalised service to reflect individual goal setting priorities and a focus on gradual and holistic change dealing with issues beyond health. The important role of a strong and supportive relationship with an easily-accessible link worker in promoting sustained behaviour change highlights the importance of link worker continuity. A lack of suitable and accessible voluntary and community services for onward referral acted as a barrier to involvement for some participants. Conclusions: This study highlights issues of interest to commissioners and providers of social prescribing. Engagement with social prescribing for up to two years was examined and continued involvement was identified for those with complex issues, suggesting that a long-term intervention is required. The availability of onward referral services is an important consideration for social prescribing in a time of constrained public spending. From a research perspective, the range of improvements and their episodic nature suggest that the evaluation of social prescribing interventions requires both quantitative and qualitative data collected longitudinally.

Lamb Street to the pod: the journey from 'service user' to citizen: a case study about Coventry City Council's award-winning Pod

This paper describes how Lamb Street Day Centre changed into The Pod, a place providing social brokerage to support and transform the lives of people with severe mental illness whilst also benefitting the wider community. Social brokerage aims to maximise an individual's connection to and inclusion in the community, and help to build social support networks. The Pod receives around 200 referrals a year, and people are supported to re-engage with their communities, access universal opportunities rather than ‘use’ services. The Pod, which is run by Coventry City Council, also hosts a café and manages a city-wide programmes, each bringing people together in a way that leads to positive social change. The paper includes short case studies which show how individuals have benefitted from the Pod.
