About the Transitions project
SCIE is working with the Department of Health and Social Care (DHSC) and Department for Education (DfE) on a new project to improve transitions into adult social care.
High-quality health and care transitions from childhood to adulthood are critical to helping prevent crisis, reduce inequalities and support developmentally appropriate independence. Improving transitions is a key test of system integration, continuity of care and effective reform.
The variation in quality of transitions from children’s services into adult social care is a widely understood problem, with many young people experiencing fragmented services, where planning and coordination between children’s and adult services is disjointed. and practice.
This project focuses on delivering national best practice guidance, training and resources that will ensure staff have the knowledge and tools they need to provide high-quality support to young people as they transition into adulthood, helping them live as independently as possible.
SCIE’s unique expertise across the breadth of both adult and children’s social care, ensures we are best-placed to support improved join-up for care, health and education at both strategic and service levels. Through bringing more collaborative strategies and shared decision-making we aim to prevent needs escalating further, leading to net savings for local systems.
The Transitions project is being co-produced, working with young people and parents/carers, social care leaders and practitioners, ensuring real experiences and expertise shape all resources.
