From place-based to place-led

A whole-area approach to integrating care systems

This report explores how partnerships at the level of local place can drive forward the ambitions of the NHS Long Term Plan, including addressing the wider determinants of health and inequalities. It provides practical lessons on a range of topics including setting the ambition and guiding principles for place-based approaches, investing in collaborative leadership, designing the right system governance and prioritising and responding to the needs of local people.

Our ambition is that the overall plan for each place feels like it is the people’s plan for the place – their plan. To enable this, we make sure that within our governance structure we have the public and third sector and NHS and council involved at the highest level.

SCIE was commissioned by the NHS Confederation to produce this report as part of a series of resources for leaders in integrated care systems and sustainable transformation partnerships to deliver the ambitions set out in the NHS Long Term Plan. It was developed with leaders from across health and social care systems with the aim of providing practical learning which can help them make progress.
