Transition from children’s to adults’ services – key resources

This pulls together all the existing resources, policy documents and initiatives that relate to transition from children’s to adults’ services.

There are three cross-cutting themes:

• becoming an adult and achieving independence
• changes in the actual services used
• explicit duties in relation to the Care Act 2014 and the Children & Families Act 2014.
