Gloucestershire County Council
Village and Community Agents are trusted members of Gloucestershire rural communities who provide face-to-face information, signposting and support to local disadvantaged and isolated people, including older people (aged 50 and over) and adults aged over 18 affected by cancer, enabling them to make informed decisions about their own wellbeing. There is an emphasis upon providing community-based solutions and on promoting independence among its older clients.

Brighton & Hove City Council
A twice-monthly group designed to support male adults in Brighton and Home who are caring for a partner, relative or friend, through coffee mornings and self-determined social activities, provided free of charge. One-to-one support is also provided by the Carers’ Centre and all activities are organised to fit around caring responsibilities and accessibility needs.

Devon County Council
Time for Life (TfL) is a free community enabling service available to older adults aged 65 and over across Devon who have become socially isolated and/ or excluded as a result of bereavement, illness, disability, carer responsibility– or a range of factors unique to their situation. It aims to support individuals to regain confidence through meaningful social activities, re-engage with the community (or engage with a new one after relocation) and build up the independent living skills to prevent isolation in the future.

Stockport Metropolitan Borough Council
Stockport’s Prevention and Personalisation Service is a person-centred recovery-focused approach to providing mental health services. By working with people to identify what they want to achieve in their lives, and to support them to overcome non-medical barriers which are hindering their recovery, the service aims to improve people’s wellbeing; reduce use of secondary services and prevent a dependency on medical care, where this is avoidable.

East Sussex County Council
Home Works is a short-term floating housing support service which is accessible for all working-age (aged 16-64) adults who feel they needed support with independent living skills, resilience and improving their wellbeing. Rather than being limited to residents of particular housing associations Home Works supports people living in a variety of circumstances (the majority renting privately or in insecure housing) across East Sussex.

Buckinghamshire County Council
Prevention Matters is a multi-component service model which aims to provide free advice and services to all adults with low to moderate support needs (which in the main currently fall below the eligibility criteria for adult social care), before personal health or social problems become critical. The four components aim to reduce and prevent social isolation and other social difficulties, develop community capacity, improve and promote the importance of volunteering , maximise the effectiveness of public resources and gather intelligence in order to provide an evidence base for further preventative work and how best to target resources in the future.

Derby City Council
Local Area Coordinators have been initially based in two Derby city ward communities and each provides support and assistance to people who may be vulnerable due to age, disability, mental health needs or sensory impairments, but who may not meet the eligibility criteria for specialist services or are not yet at crisis point. They provide two levels of support – information, advice, advocacy and low level support at level 1; level 2 support is a longer-term relationship where they help vulnerable people describe and then pursue their vision of ‘a good life', making best use of the individual and community resources around them, choosing local or non-service solutions where possible.

Thurrock Council
Local Area Coordination helps vulnerable people in Thurrock find support in their local communities. It forms part of Thurrock's Building Positive Futures programme, which aims to support older and vulnerable people to live well; to increase their health and wellbeing; improve housing and neighbourhoods and create stronger, more hospitable and age-friendly communities. LAC was selected as an approach because more than 20 years of evaluation of its use in Western Australia has shown it to lead to positive outcomes for individuals, build community resilience and reduce demand for formal services.

East Sussex County Council
STEPS to Stay Independent is a short term housing support service provided to older people in East Sussex, enabling them to retain or regain independent living skills so they can continue to live in their own home or move to one that better meets their needs. It is part of the Supporting People programme in East Sussex, alongside another preventative programme Home Works, and extra care and supported housing.
