The Heart of Living and Dying
brings a social work and public health approach to Advance Care Planning. Developed by Deirdre McKenna a social worker in the Southern Heath and Social Care Trust in Northern Ireland this process promotes the value of group work in supporting people to reflect on what matters most to them in life and in planning for death and dying. Communicating effectively in these circumstances is vital and enabling people to discuss what is important to them has been shown to enhance people’s quality of life and peace of mind. This publication offers social workers and other health and social care professionals a guide to facilitating these important conversations amongst loved ones, families, friends, groups and communities.
