BCST Webinar: Using the Integrated Logic Model: 07 December 2018 Open
07 December 2018
11.30am - 12.30pm. Local areas need to develop plans and commission services to integrate care based on the very latest evidence of what works. They also need to understand to what extent integrated care is being delivered, and most importantly, whether people’s experiences and care outcomes are improving. The logic model for integrated care developed by the Social Care Institute of Excellence (SCIE) describes what good looks like, providing a visual depiction of how a fully integrated health and care system might be structured and function, and the outcomes and benefits it should deliver for those who use services and their carers. This online seminar delivered by SCIE on behalf of the Better Care Support Team (BCST) will focus on how local areas can use the logic model as a tool to plan, monitor and evaluate the success of your integrated schemes. It will be run by SCIE Associate Deborah Rozansky.
